Confidentiality
All services provided are confidential. No information will be released without the written permission of the student. A "Medical Records Release" can be obtained in Student Health Services, or the student may mail or fax a request. In the event of the latter, the request should include the student's name, social security number, records needed, the program attended (undergraduate/graduate), and the date of entry and/or graduation.
Often, parents will want to discuss the student's medical condition, or students themselves will want their parents involved. A Medical Records Release is needed for each illness or injury in order to protect the privacy of the student.
